National Healthcare Decisions Day Set for April 16, 2009

Newton – April 2, 2009- Karen Ann Quinlan Hospice, along with other national, state and community organizations, are leading a massive effort to highlight the importance of advance healthcare decision-making—an effort that has culminated in the formal designation of April 16, 2009 as National Healthcare Decisions Day (NHDD). As a participating organization, Karen Ann Quinlan Hospice is providing information and tools for the public to talk about their wishes with family, friends and healthcare providers, and execute written advance directives (healthcare power of attorney and living will) in accordance with NJ & PA state laws.

Specifically, on April 16, from 9:00 am to 4:00 pm, Karen Ann Quinlan Hospice and Home Health Care is welcoming the public throughout the day with free information about advance care planning and advance directive forms at their three locations. These resources will be available at the Karen Ann Quinlan Hospice office located at 99 Sparta Ave, Newton, the Karen Ann Quinlan Home Health Care office located at 755 Memorial Parkway, Suite 303A, Phillipsburg, NJ and in PA, 315 West Harford St. Milford

“As a result of National Healthcare Decisions Day, many more people in our community can be expected to have thoughtful conversations about their healthcare decisions and complete reliable advance directives to make their wishes known,” said Dr. Roxanne Debski-Seigel, Director of Marketing and Advancement for Karen Ann Quinlan Memorial Foundation. “Fewer families and healthcare providers will have to struggle with making difficult healthcare decisions in the absence of guidance from the patient, and healthcare providers and facilities will be better equipped to address advance healthcare planning issues before a crisis and be better able to honor patient wishes when the time comes to do so.”

For more information about National Healthcare Decision Day, or to download advance directive forms for your state please visit www.nationalhealthcaredecisionsday.org.

Karen Ann Quinlan Memorial Foundation is passionately dedicated to providing Hospice care for the terminally ill, Bereavement for those who have lost loved ones and Home Care assistance for recovery patients. The Future of Nursing Education is Still a Controversy

The Future of Nursing Education is Still a Controversy

By: Roxanne Debski-Seigel, Ph.D.

As we come to the close of national nursing week (May6-12th), those looking to enter the nursing field are once again faced with the debate revolving around what should be their minimum educational requirement. Dating back to 1965 when a position paper was published by the American Nursing Association calling for the BS (Bachelor of Science) being the minimum requirement for entry into professional nursing practice, this controversy continues today to frustrate and divide those in and associated with the field.

The debate seems to boil down to one of technical knowledge verses academic knowledge, and what will be necessary to face and keep up with the advancements and the complications of modern medicine as practiced today and going forward. In one area, the importance of developing more well-rounded educated nurses as a profession seems reasonable going forward, but then the practical side of the current shortage rears its head. And historically, when shortages become severe, necessary credentials have slackened. But according to many in the industry, the so called relaxed versions are fundamentally acceptable with respect to patient care, critical thinking and decision making. Many say the hands-on experience and teachings of a 4 year hospital based nursing certificate program or Associates two year program deliver what patients need. “Considering that an average 4 year Bachelor’s degree is 2 years of your major course study and 2 years of “rounding” course work, then it stands to reason a 2 year AAS in nursing is equal professional training-wise to a 4 year BSN acknowledged Darrin Adams, RN, LPC and Karen Ann Quinlan Hospice administrator. He continued “So as far as a BSN advancing the nursing profession, I just don’t buy it.”

But the other side states the Bachelor of Science Nursing degree from a traditional four liberal education institution is what is needed to advance the profession. And to carry the controversy further, those who have received their RN’s will not be grandfathered into their profession, but will have to pursue and complete their BSN to practice. While in NJ there is a period of ten years proposed to complete this, it still constitutes a hardship for those in the field who now may have other obligations and do not have the finances or time to chase a four year college degree. This may result in many nurses leaving the field or pursue other careers, and so the shortage grows. The opposite of this view is that because BSN’s have not been required, there is now also a shortage of nursing teachers; for there is a big gap between the 2 year degree and an advance degree.

Our health population is growing in two basic ways; the population where the most advanced technology will be used to treat and the aging population where such programs as hospice are used when curative treatments are not being pursued. This is not suggesting that one segment should receive better care than the next, but rather, the trained nurse who does not have the BSN, many in the industry argue, is as competent in providing the necessary patient care.

So where does this controversy put the industry? Should the states care about a shortage? Are hospitals willing to pay what should be appropriate for those nurses who have incurred the expense of a four year college degree? Should then RN’s be grandfathered, but receive a lesser pay than BSNs.

The introduction of on-line schools have eased the entry for those who have decided to continue for their BSN; for many of us who achieved their Bachelors degree and further, have paid that price, so why not a profession who is directly involved with our medical care?

North Dakota has required all new nurse hires to possess a BSN degree since 1987. The New York State Board has similar legislation pending. The Dept. of Veteran’s Affairs began requiring all new hires to possess at least a BSN degree in 2005. In New Jersey there is pending legislation that would require RN’s to obtain their BSN within ten years in order to practice their profession. Who knows what’s coming next?

“Rapidly expanding clinical knowledge and mounting complexities in health care mandate that professional nurses possess educational preparation commensurate with the diversified responsibilities required of them. As health care shifts from hospital-centered, inpatient care to more primary and preventive care throughout the community, the health system requires registered nurses who not only can practice across multiple settings – both within and beyond hospitals – but can function with more independence in clinical decision making, case management, provision of direct bedside care, supervision of unlicensed aides and other support personnel, guiding patients through the maze of health care resources, and educating patients on treatment regimens and adoption of healthy lifestyles. In particular, preparation of the entry-level professional nurse requires a greater orientation to community-based primary health care, and an emphasis on health promotion, maintenance, and cost-effective coordinated care.” (American Association of Colleges of Nursing, Position Statement, Dec. 2000)

“As a nurse for ten years I have seen the changes occurring in the healthcare field. I know we must increase our education levels. As a profession we need to advocate for our expertise, our knowledge and our skill.” stated Beth Sylvester, RN and Karen Ann Quinlan Hospice Nurse Supervisor.

For now there have been no satisfactory solutions presented that appease everyone, and the controversy continues while our nursing and shortage grows and our health care becomes more demanding.

Karen Ann Quinlan Hospice and Congressman Scott Garret’s office make a patient’s wish come true.

Nelly Tafferelli came to this country from Argentina in 1992 and dreamed of becoming a U.S. citizen ever since.

Her efforts, even when assisted by friends and family, weren’t enough. Thanks to the team from Karen Ann Quinlan Hospice: Jackie Shepard, Social Services, Marlina Schetting, Social Services Director and Nancy Lombreglia, RN, Nelly’s dream is now a reality. On her behalf they contacted Congressman Scott Garrett and with the help from his office some of the red tape got unstuck.

She passed her citizenship test and attended her swearing in ceremony all while wearing pink pajamas!

Due to Nelly’s terminal illness, Officer Steve Sprinkler of the U.S. Citizen and Immigration Services administered the citizenship test to her at home where she was able to answer questions in comfort and confidence.

Afterwards, she celebrated and blew out eighteen candles (one for each year here), ate some cheesecake, and sang “God Bless America”.

“At last I am an American!” she chortled in delight.

The Karen Ann Quinlan Hospice has been taking care of terminally ill patients and their families for over a quarter of a century. With three offices; Newton and Phillipsburg, NJ and Milford, PA they are licensed in NJ & PA and serve North and Northwest NJ and the Pike County area of PA. In addition to hospice the Karen Ann Quinlan Memorial Foundation provides bereavement and home health care. Joseph T Quinlan Center Open House Welcomes Community.

The idea of a Joseph T Quinlan Bereavement Center grew out of the desire to provide the community, in addition to hospice families, grief support for loss of loved ones.

Before the first center was opened, there was a history of bereavement counseling provided by Karen Ann Quinlan Hospice as part of the hospice benefit. Cecelia T. Clayton, now Executive Director of the hospice, was then serving as Volunteer Coordinator and the initiator of many bereavement groups in existence today. Seeing a further need in the community, Clayton presented a proposal to the Hospice Board of Directors to create The Joseph T. Quinlan Bereavement Center. By acting on the opportunity of donated space from St. James Episcopal Church in Hackettstown, the hospice board was assured the bereavement programs would be available not only to hospice families, but to anyone in the community who was grieving a loss through death.

The center also held individual counseling and support groups for Sussex County at the Karen Ann Quinlan Hospice offices located at 99 Sparta Ave, Newton. The area’s demand for grief counseling for both adults and children has increased over the last ten years since the first center was opened in 2000 and they quickly outgrew the hospice office space that was providing local programs.

At the beginning of this year, The Joseph T. Quinlan Bereavement Center opened a second location on 61 Spring St., Newton. Director of Bereavement, Diana Sebzda, MA, LPC, CT, has put into practice many additional programs and activities.

The services provided at the center will consist of anticipatory grief and bereavement programs which are offered to hospice families and the entire community.

Included are:

  • Coping With Loss Support Groups
  • Individual Grief Counseling Sessions
  • Children’s Art Bereavement Programs
  • Pet Loss Support Groups
  • Healing in the Classroom workshops for our schools
  • Movie Nights and Bereavement Facilitator Training Series.

Also for this event, their will be a “butterfly” art display and silent auction. The “butterfly” is the symbol of transformation and majestically honors the life cycle with its impressive process of metamorphosis and has served as the symbol for grief and hospice.

The Sussex County Arts and Heritage Council has donated an original butterfly art piece and all original works displayed will be available via a silent auction held that evening and proceeds will benefit both the Arts Council and the Bereavement Center.

If you would like to get to know the Joseph T Quinlan Bereavement team please join them at their Grand Opening and Open House celebration to be held Wednesday, March 30th from 4 to 7:00 p.m. Parking is available on the Trinity St entrance and the bereavement center can be accessed via the Trinity Street ground floor level or for handicap access via the Spring Street entrance and take the elevator to the 1st floor.

The center is a non-profit and is able to offer this care through donations and grants from our clients and the community; no one is ever turned away regardless of ability to pay.

Spirituality and End of Life Care

Each year the Hospice Foundation of America (HFA) presents a nationally recognized distance learning program to more than 125,000 people in 2,000 communities. For more than a decade, this annual educational event has been instrumental in educating healthcare professionals and families on issues affecting end-of-life care. The program provides an opportunity for a wide variety of professionals – including doctors, nurses, funeral directors, psychologists, educators, social workers and bereavement counselors – to share and exchange ideas and obtain continuing education credits.

This year HFA’s 18th Annual National Bereavement Program focuses on “Spirituality and End-of-Life Care.” The program will discuss differences between spirituality and religion, while also addressing spirituality during illness, death and grief; spiritual assessment and empowerment, and life review. Moderated by Frank Sesno, Director of the School of Media and Public Affairs at The George Washington University, the program will be shown at The Joseph T. Quinlan Center-Sussex County, 61 Spring St. Newton, April 13th from 1 pm to 4:00 pm. Sign in starts at 12:30 and the program ends with a panel discussion from 4-4:30.

Mr. Sesno will lead the panel of noted authorities that includes: Kenneth J. Doka, Professor of Gerontology at the Graduate School of the College of New Rochelle; Gary S. Fink, Chaplain and Dementia Project Coordinator at Montgomery Hospice, and Adjunct Faculty at Hood College Graduate School; Carolyn Jacobs, Dean and Elizabeth Marting Treuhaft Professor at Smith College, School for Social Work; Betty Kramer, Professor at the University of Wisconsin-Madison, School of Social Work; Reinette Powers Murray, owner of The Peaceful Journey End-of-Life Process Program, and certified as a Train-the-Trainer for End-of-Life Nursing Education (ELNEC); and Martha Rutland, Director of Clinical Pastoral Education at VITAS Innovative Hospice Care.

Each year this award-winning, program is produced by Hospice Foundation of America, a not-for-profit organization, which acts as an advocate for the hospice concept of care through ongoing programs of professional education, public information and research on issues relating to illness, loss, grief and bereavement.
